Amy's (ex-)husband, Michael Halterman, is included in the cast. Over time the show has grown to include other members of the Slaton family, such as Chris, Misty, and Amanda.[3] Chris Combs,[4] Amy and Tammy's brother, has also decided to lose weight on the series and continues to do so alongside both his sisters. He accompanies Tammy to her doctor visits.[5] The show has also featured Amy and Michael Halterman's sons Gage Deon Halterman & Glenn Halterman.[6]

In the small town of Dixon, Kentucky, sisters Amy and Tammy Slaton live together with Amy's husband Michael Halterman. Amy makes dinner for her and Tammy which includes many unhealthy foods. Amy reveals she has many conditions due to her weight including diabetes. Tammy also reveals that she has many medical conditions, she has been to the hospital numerous times and has been on life support twice.
